Sheppard Dysphagia Institute Terms and Conditions


Please read these terms and conditions of use carefully before accessing, using or obtaining any Sheppard Dysphagia Institute, LLC (SDI) products or services. By accessing this SDI course you agree to be bound by these terms and conditions.


Eligibility

This certification/training is open to practicing and student Speech Language Pathologists (SLP) and practicing Physical Therapists (PT), Occupational Therapists (OT), Registered Nurses (RN) and other people trained in making clinical observations and scoring percentiles. 


Cancelation and Usage Policy

To access this certification training you will pay $200 USD through PayPal.  A PayPal account is not required.  If for any reason you are unable to pay through PayPal, please contact shepparddysphagia@gmail.com.  

Once you begin this certification training, no refund will be given.  Ensure that you are qualified to take this training by reading the previous Eligibility section.  Once you enroll you will have 90 days to complete this course.  Extension of your enrollment period is available for free upon request.  

After you have completed the DDS/DMSS Certification Training you will be un-enrolled and no longer have access to the course or its content.  To extend your enrollment duration, access course materials or for any other needs or questions, please contact shepparddysphagia@gmail.com.


Certification

By enrolling in the DDS/DMSS Certification Training you are purchasing access to it on Moodle, a learning platform.  Upon successful completion of the course you will receive an email with a link to your certificate.  Certification is conditional on you completing and passing a competency assessment at the end of the course.  The Sheppard Dysphagia Resource DDS/DMSS Certification Training certifies lifetime use of the suite of assessments covered in the training.  SDI does not warrant that participation in this course will guarantee successful completion of the examination. 


Responsibilities

With this training, Sheppard Dysphagia Institute, LLC (SDI) intends to teach you how to use the DDS/DMSS assessment.  SDI does not warrant that this program or its examination certifies a candidate’s competence or ability to evaluate or treat any disorder. Furthermore, nothing about this program or its examination is intended to replace, override, or conflict with other licensing requirements. 

The assessor maintains responsibility for their own observations and judgments regardless of certification. Clinical outcomes are the responsibility of the individual conducting the DDS/DMSS.  Recommendations for treatment should only be made by those qualified to make recommendations regarding feeding and swallowing disorders.


Usage restrictions

The contents of this training, DDS/DMSS User’s Guide and all forms therein are copyrighted. You are allowed to download and print the forms and User's Guide, but only for your own use.  Distribution or sharing content of this training is not permitted.  The downloading, copying, or saving of videos, text, or images is strictly prohibited.

If you wish to create your own digital versions of our forms contact shepparddysphagia@gmail.com for permission.  You must include written instructions on each digitized document stating clearly that this form can only be used by those who have been certified in the DDS/DMSS by SDI.  If you wish to upload forms filled in with patient information, observations, and scoring to an electronic dossiers system, you are welcome to do so.
